Facility has� solids generated during the neutralization process that is thrown into the garbage <br /> without making a hazardous waste determination. Immediately make a determination of the status of <br /> the waste and if hazardous dispose it according to title 22 HW regulations. Provide proof of corrective <br /> action to this office by 6/27/10. <br /> #8. Facility has a inactive EPA ID number. Immediately, activate the EPA ID number by filling out a <br /> DTSC 1358 form provided. Submit proof of corrective action to this office by 6/27/10. <br /> #20. 3 - 5 gallon bucket and a spill rack containing waste was not labeled with hazardous waste. <br /> Pursuant to California code of regulation section 66262.34(f) a facility must label their HW container with <br /> at least the following information, the word "Hazardous Waste", name and address of generator, <br /> accumulation start date, physical state, hazardous properties and content. Begin practice immediately. <br /> Provide proof of corrective action. <br /> #27. A spill rack used to store HW was not closed. Pursuant to CCR section 66265.173(a) a HW <br /> container must be closed at all times except when adding or removing HW. Provide proof of corrective <br /> action. <br /> #52. Facility lack emergency coordinator and a contingency plan. Immediately appoint an emergency <br /> coordinator and prepare a contingence plan for the facility and post it by a phone readily available to <br /> employees. <br /> #83. Facility failed to label universal waste to identify the type of universal waste. Immediately label all <br /> universal waste to identify the type of universal waste. Provide proof of corrective action to this office by <br /> 6/27/10. <br /> Notes <br /> A copy of complete return to compliance certification must be submitted to this office by 6/27/10. <br /> Provided HW label and labeling instructions. <br /> Provided free CUPA class information <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector: Muniap a Naidu Received By: Title: <br /> A4 , -c 0_� <br />
