The consolidated manifesting receipts contain a temporary (one time use) EPA ID number for J & C <br /> Mobile Truck Repair (CAC002761968). The receipt must contain a permanent EPA ID number for the <br /> facility. All copies of each receipt from a consolidated manifesting procedure shall contain all of the <br /> following items: <br /> 1. name, address, EPA ID number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative <br /> 2. date of the shipment <br /> 3. manifest number <br /> 4. volume or quantity of each waste stream, its California and RCRA waste codes, the waste stream <br /> type, and its proper shipping description, including hazardous class and UN/NA identification number <br /> 5. name, address, and EPA ID number of the authorized facility to which the hazardous waste is <br /> being transported <br /> 6. transporter's name, address, and EPA ID number <br /> 7. the driver's signature <br /> 8. a statement signed by the generator, certifying that the generator has established a program to <br /> reduce the volume or quantity and toxicity of the hazardous waste to the degree as determined by the <br /> generator to be economically practicable. <br /> Ensure that future receipts from all consolidating manifesting procedures for any hazardous wastes are <br /> properly completed with the correct information. <br /> 301. Oily staining was observed on the ground in the yard north of the facility. Facilities shall be <br /> maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to <br /> air, soil, or surface water which could threaten human health or the environment. Immediately clean up <br /> all stained soil, and manage according to Title 22 hazardous waste regulations. Submit a statement and <br /> supporting documentation explaining how this waste was managed by October 3, 2014. <br /> 605. A 55 gallon blue and white steel drum of used antifreeze and three 55 gallon blue and white steel <br /> drums of used oil were observed in the hazardous waste storage area with only the contents labeled. All <br /> hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" 4. physical state (liquid, solid) <br /> 2. name and address of generator 5. composition (contents) <br /> 3. hazardous properties (ignitable, corrosive, reactive, toxic) 6. accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all <br /> the required information. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Stacy Rivera (209) 468-3440 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
