Per the Well &Boring <br /> Permit Application Requirements, the following is included: <br /> *Work Plan <br /> *Check for Site Mitigation Fee, Inspection Activity Fee, and Permit Application Fee <br /> *Well &Boring Permit Application and Licensed Contractors Declaration <br /> I have a few questions about the Master File Record Information Form. So, that is not included. <br /> I wanted to get everything else to you so you can start reviewing it, as we have the contractors <br /> scheduled to do the field work the week of August 5.
