Box 2009 <br />Stockton, CA 95201 <br />Dear Ms. Khanna: <br />Attached is the annual Hazardous Waste generator tax payment for the reporting <br />year ending December 31, 1990, for the Libbey-Owens-Ford Co.'s (LOF) facility <br />located at 500 East Louise Avenue in Lathrop, California. The total fee due for <br />this time period is $1212.50. <br />LOF is paying these fees to the San Joaquin Local Health District as LOF <br />understands that the District has been granted, under the jurisdiction of the <br />California Department of Health Services, the authority to assure compliance <br />with the State Hazardous Waste Control Law (California Health and Safety Code, <br />Division 20, Chapter 6.5). Also, the San Joaquin Local Health District has <br />established hazardous waste generator fees equal to those charged by the State <br />Board of Equalization.
