The facility's contingency plan is incomplete. Upon review of the plans provided at <br />time of inspection a listed emergency coordinator could not be found. The contingency plan must include: <br />1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non -sudden <br />release of hazardous waste to air, soil or surface water at the facility; <br />2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br />State and local emergency response teams to coordinate emergency services; <br />3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br />coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all <br />others in order in which they will assume responsibilities; <br />4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities, <br />5. Evacuation routes for facility personnel where there is a possibility of evacuation, <br />6. Current telephone number for State Office of Emergency Services. <br />If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency plan, <br />the owner or operator need only amend that plan to incorporate hazardous waste management provisions that are <br />sufficient to comply with the requirements of this chapter Immediately prepare or amend an existing plan to fulfill the <br />requirements of this chapter. Submit a copy of the updated portion of the contingency plan to the EHD. This is a minor <br />violation. <br />605 <br />Failed to completely label containers or tanks of hazardous waste. 1 rolloff bin was seen without the hazardous <br />property marked. A second roll -off bin was seen without a label. All hazardous waste containers shall be marked with <br />the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. <br />This violation was corrected on site during the inspection. This is a minor violation. <br />Overall Inspection Comments: <br />Complete a submit a copy of the Return to Compliance certification with a statement addressing how each of the <br />listed violations have been corrected to this office within 30 days, October 30 2014. Submit copies of any <br />requested documentation. <br />Permission to conduct inspection, take samples and take photos was granted by Tim Gutierrez, Terminal <br />Operator. This facility generated 0.2150T of hazardous waste in 2013. Over the past 12 months they have <br />generated 10.875T of hazardous waste. Currently there are 2-20 yard bins of hazardous waste being used by <br />contractors on site. The waste will be removed shortly after the completion of the job.
