The contingency plan for this facility did not include the list or location of decontamination <br />equipment. Although this facility is currently a small quantity generator, they have opted against the use <br />of a modified contingency plan at this time. A contingency plan shall include all of the following: <br />- actions facility personnel shall take in response to fires, explosions, and unplanned release to air, <br />soil, or surface water <br />- arrangements agreed to by local police departments, fire departments, hospitals, contractors, and <br />state & local emergency response teams <br />- names, addresses & phone numbers (office & home) of the Emergency Coordinator <br />- list of all emergency equipment at the facility (fire, spill, communications/alarms, decontamination), <br />including location, physical description, and a brief outline of its capabilities <br />- evacuation plan, including routes and signals to be used <br />- telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated portions of the plan <br />to the EHD by November 4, 2011. <br />812. According to Mr. Eikmeier, a -waste is being taken to a recycler; the recycling records do not list any <br />e -waste. A generator of universal waste (alkaline batteries, mercury containing equipment, fluorescent <br />bulbs, electronic devices, CRTs, aerosol cans) shall keep a record of each shipment of universal waste <br />on site for at least three years. The record may take the form of a log, invoice, manifest, bill of lading, or <br />other shipping document, and shall include the following: <br />- Name & address of destination facility <br />- Quantity & type of universal waste <br />- Date of departure/shipment <br />Begin this practice immediately. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by November 4, 2011 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />"Facility generated 1.48 tons of hazardous waste in 2010. The facility will be moved to a lower permit <br />category (<5 tons/year).
