Follow-up reports will be submitted to the following agencies (Refer to Section 5.8 for specific <br /> reporting instructions): <br /> COUNTY OF SAN JOAQUIN, OFFICE OF EMERGENCY SERVICES <br /> 222 E Webber Avenue, Room 610 <br /> Stockton, CA <br /> (209)468-3969 <br /> NATIONAL RESPONSE CENTER <br /> 2100 Second Street SW <br /> Washington D.C. <br /> (800)-424-8802 <br /> 5.6 Public I External Notifications <br /> If there is a spill that may affect the surrounding community, Leprino Foods and local emergency <br /> responders should be aware of the residences and businesses, which may be affected by a release. <br /> Refer to Table 1-1 for definition of reportable quantities. <br /> Table 1-2 is provided for the purpose of identifying sensitive receptors within a 1.7 mile radius(distance <br /> to end point) of the plant. This calculation is based on the end result of our Worst-Case Scenario, a <br /> complete release(within 10 minutes)of the High Pressure Receiver. The Alternate Scenario results in a <br /> distance to end point of 0.2 miles. No sensitive receptors are represented in this close proximity. <br /> 5.7 Press Relations <br /> If you are contacted by the media for comment on any incident, politely decline comment and assist the <br /> caller in reaching the office of the Plant Manager or Western Region Vice President. The following is an <br /> example of a recommended response: <br /> "Our company is organized to handle media inquiries accurately and quickly. I will <br /> report your call immediately to the Plant Manager or Western Region VP." <br /> 5.8 Incident Documentation/Reporting Documentation/ReportingRequirements <br /> A written confirmation report may be required by the US Environmental Protection Agency(USEPA),as <br /> soon as practicable, after the reporting of a discharge. The Plant Manager and Corporate offices <br /> should review and approve this report for accuracy prior to submission to the EPA. This written <br /> confirmation report shall include: <br /> • The name, address, and telephone number of the individual that reported the release; <br /> • The name, address, and telephone number of the individual submitting the confirmation report (if <br /> different from the individual reporting the release); <br /> • The source of the release; <br /> • The location of the release, as follows: <br /> — For discharge from sites located on land; the name of the site, the street address, the tax parcel <br /> #213-070-050,the municipality,the county,and any state or EPA ID numbers of facilities involved; <br /> or <br /> — For discharge on,under, or into water;the name of the water body,the latitude and longitude of the <br /> place the discharge originated, and a map identifying the areas affected by the discharge; <br /> • A list of the chemical name and Chemical Abstract Service(CAS)number and CHRIS Code of each of <br /> the hazardous substances discharged; (Ammonia: CAS#7664-41-7, CHRIS Code `AMA') <br /> • A fist of the quantities of each hazardous substance discharged, including best estimates if the <br /> quantities are unknown; <br /> • The dates and times for the following: <br /> — discharge began <br /> 11Lep_440%userslpsmtelemenl 12 emergency pian and responseMacy Emergency Preparedness and Response PEan.doc Rev,July 16,2002 <br /> Section 5, Page 4 <br />
