A <br />FIRST STRIKE El�TAL <br />SPILL RESPONSE SERVICE MENT CONTRACT <br />First Strike Environmental (FSE) shall provide 24 -Hour emergency response services. <br />> The phone number for FSE's Environmental Response Dispatch. Center is, 1-800447-3558. <br />Work to be performed within the geographical boundaries of Oregon, Washington, and Northern <br />California. <br />> Response to hazardous waste.incidents, including but not limited to, oil/diesel spills, shall be within a <br />reasonable length of time or as mutually agreed upon by both parties. <br />FSE shall, upon approved and authorized notice, dispatch appropriate personnel to perform emergency <br />response services as needed to control, contain, clean up, transport, and dispose of hazardous materials <br />in the most efficient and cost-effective manner. <br />FSE shall contact the responsible party or their insurance carrier directly for issuance of the work <br />order. All billings by FSE will be sent directly to the client or their insurance carrier as requested. <br />> FSE shall complete an Initial Spill/Release report, Final Emergency Response and Cleanup Report, and Site <br />Closure report, where applicable, submit copies of laboratory. analysis, and hazardous waste disposal <br />information at the time of site closure as necessary. <br />• > FSE shall maintain customer confidentiality at all times. <br />This agreement does not limit Food Express, Inc. to FSE environmental services. <br />> FSE's Spill Response Services Agreement will satisfy the requirements that a company provide evidence of <br />contract with private spill responders, identity of response personnel, telephone numbers to response <br />personnel and applicable state agencies, a description of the information to be presented to the spill <br />contractor, and the procedures -for site personnel during a spill if inserted into company's written <br />emergency response plan. <br />> The fee for FSE's Spill Response Services Agreement during this contract period will be $ 200.00. <br />Food Express, Inc. agrees to the rates quoted on FSE's current rate sheet. <br />IN WITNESS WHEREOF, the parties have executed this agreement as of the day and year first written herein <br />below. <br />CONTRACTOR <br />CLIENT COMPANY <br />Mark Dillow Food Express Inc. <br />-T� E�✓ <br />Dated: <br />6 or 6.
