Be sure to include <br /> your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant <br /> activities and required fields. <br /> Notes: A verbal consent to inspect, photograph and sample was given by Shawn Harbin, Mechanic. Jimm <br /> Vosburgh, Vice President, was also present during the beginning of the inspection. Amount of hazardous waste <br /> generated from last calendar year could not be determined since no disposal records were available to review at <br /> the time of the inspection. The facility has a 3-componment clarifier and the sludge has been pumped out in 2015 <br /> Also, observed a sandblaster or bead blaster at the facility, which is not being used at the time due to broken <br /> glass. The facility will replace the glass on the sandblaster or beadblaster and use it in the future. Discussed <br /> hazardous waste determination for sandblaster or bead blaster waste media, hazardous waste labeling, storage <br /> time, closed container, training, management of empty containers, retention of disposal records for3 years and <br /> manifesting procedures. <br /> Provided: Return to compliance form, modified contingency plan template form, hazardous waste labeling <br /> requirements and hazardous waste labels and a list of free CUPA courses <br /> Waste Streams: used oil, used absorbent and spill pads, sludge from 3-compartment separator and antifreeze <br /> Reviewed Documents: no documents <br /> The following are not violation(s): <br /> #103-No modified contingency plan <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of <br /> fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. This was corrected on site. <br /> #403-Failed to keep hazardous waste containers closed except when adding or removing hazardous waste <br /> Observed a 5-gallon container of used antifreeze stored open. All hazardous waste containers shall be closed at <br /> all times except when adding or removing waste. Immediately close these containers and ensure all hazardous <br /> waste containers are closed when not adding or removing waste. This was corrected on site. <br /> Page 5 of 6 <br />
