Site Safety Plan • • <br /> Job No. 4780201 Pa e S <br /> Based on the hazards listed above, the minimum level of personal protection <br /> ' required on this site is a modified Level D. This hazard determination requires <br /> that approved air purifying respirators with appropriate cartridges be available,. <br /> to individuals involved with site investigation operations. Respirators will only <br /> ' be worn if air-monitoring indicates that respiratory action levels have been <br /> exceeded. This hazard level also requires that dermal protective equipment be <br /> used when there is a high probability of skin contact with contaminants. <br /> ' Modified Level D for the purposes of this project will include the following PPE: <br /> ' - Coveralls/work uniform; <br /> ' - Hard hat; <br /> - Latex gloves when coming in contact with potentially <br /> ' contaminated soil; <br /> - Boots with approved steel toe protection; <br /> ' - Approved safety glasses or chemical splash goggles if the <br /> potential for splash exists; and <br /> ' - Approved Half-face respirator equipped with organic vapor <br /> cartridges (available for immediate use should air-monitoring <br /> ' Action Levels be exceeded). <br /> tRespirators will be NIOSH/MSHA approved, and all individuals wearing <br /> respirators will have received a documented qualitative fit test, issued <br /> ' specifically for the respirator in use. Each worker will perform a <br /> positive/negative fit test prior to each respirator usage. All personnel will be <br /> responsible for cleaning their own respirators and changing cartridges at least <br /> ' once a day. If odor breakthrough is detected while wearing the respirator, or <br /> breathing becomes difficult, leave the Exclusion Zone immediately and change <br /> cartridges before reentering.
