American Street, Stockton <br /> On March 31, 1989, the San Joaquin Local Health District was <br /> requested to witness the removal of a 8, 000 gallon ground <br /> nder <br /> storage tank at the above referenced facility. The underground <br /> removal <br /> did not occur due to the following major violations : <br /> 1• An inadequate amount of carbon dioxide (dry ice) <br /> inert the tank of flammable vapors. was used to <br /> I <br /> 2• Inadequate equipment was provided for removing the UGST <br /> from the excavation in a safe manner. <br /> 3• The associated piping from the UGST system was <br /> as required in the approved closure plan. not removed <br /> 4. The dispenser for the 8, 000 gallon UGST was apparently <br /> removed from its- Original location illustrated on the <br /> approved closure plan.
