Anderson St., Stockton, California <br />Client Name: FTG Construction Materials, Inc. <br />Project Number: 554 <br />Date: December 17, 2004 <br />General Work Description: Advance three soil borings to approximately 30 feet bgs, collect <br />soil and groundwater samples to investigate soil and groundwater conditions. <br />SSP Prepared by: Greg Stahl <br />On-site Safety Officer: Joe Vasquez <br />Project Safety Officer: Greg Stahl <br />This site safety and health plan (SSP) must be kept on-site and accessible to all on-site employees <br />during all phases of site operations. The SSP must be reviewed and signed by all on-site <br />employees, subcontractors, and visitors prior to entering the site. <br />The provisions set -forth in this SSP shall apply to Ground Zero Analysis, Inc. (GZA) employees <br />and any subcontractors working for GZA at the job site. All personnel working for GZA at the <br />job site must read this SSP, and sign the attached Compliance Agreement before entering the <br />work area. Field personnel may deviate from the safety provisions set -forth in this SSP, but only <br />to upgrade or increase the safety requirements. GZA personnel may suspend work if unsafe <br />working conditions are identified or observed, or unauthorized modifications to the safety <br />provisions set -forth in this SSP are made. If changes in site or working conditions require <br />changes in safety procedures, appropriate amendments to this SSP will be provided by the GZA <br />project manager or supervisor, with the written approval of a GZA branch safety officer or the <br />GZA Health and Safety Director. <br />PAGE 3 <br />
