ORAM
SYS I V/J
September 6, 2006
Jay Stemley
7-11 Area Facilities Manager
Mkts. 2235, 2237
RE: Disposal of Lamps and Other Waste Products
Dear Mr. Stemley,
In recognition of your requirements to explain and provide sufficient documentation
about the disposal of lamps (Universal Waste) and other Non-Universal waste products
we provide the following information and report. All lamps removed from fixtures at
your locations during the scheduled maintenance inspections or group relampslretrofits
are recycled by Sylvania Lighting Services (SLS) approved recyclers. Other waste
products that are not Universal Waste, ie, non-PCB Ballasts and Fixtures are disposed of
in accordance to local ordinances or state statute.
Osram Sylvania Inc. (OSI), our parent company audits and approves lamp recyclers for
SLS. The OSI audit includes; reviews of recycler compliance history, recycler worker
safety and training, recycler closure plan and fund, recycler certificate of insurance,
recycler laboratory tests of compliance sampling. Sylvania Lighting Services (SLS)
cannot utilize lamp recyclers that are not approved by OSI.
All removed lamps from your locations are brought back per State Universal Waste
regulation to the branch facility and sorted into the appropriate containers. The amount of
product in linear feet/units is noted on each SLS Service Service Control Form (SCF) as
well as the location it was removed from. For your convenience we have supplied a
recycling report that summarizes all recycled lamp activity. Non-universal waste items
such as non-PCB ballasts or fixture housings may be disposed of properly on site
depending on the local requirements for this waste.
Your removed lamp material is consolidated with other removed lamps from other
service customers. At specified time intervals a SLS approved recycler then removes all
lamps for appropriate recycling at the branch. The branch will subsequently (as a result of
recycling)receive a destruction certificate that summarizes the recycling activity in the
linear feet/units processed. Other Non-Universal waste products will be disposed per
customer, state or local requirement.
OSRAM SYLVANIA
100 Endicott Street
Danvers,MA 01923
W (97$)777-1900
wwwsylvania.00m
