ORAM <br /> SYS I V/J � <br /> September 6, 2006 <br /> Jay Stemley <br /> 7-11 Area Facilities Manager <br /> Mkts. 2235, 2237 <br /> RE: Disposal of Lamps and Other Waste Products <br /> Dear Mr. Stemley, <br /> In recognition of your requirements to explain and provide sufficient documentation <br /> about the disposal of lamps (Universal Waste) and other Non-Universal waste products <br /> we provide the following information and report. All lamps removed from fixtures at <br /> your locations during the scheduled maintenance inspections or group relampslretrofits <br /> are recycled by Sylvania Lighting Services (SLS) approved recyclers. Other waste <br /> products that are not Universal Waste, ie, non-PCB Ballasts and Fixtures are disposed of <br /> in accordance to local ordinances or state statute. <br /> Osram Sylvania Inc. (OSI), our parent company audits and approves lamp recyclers for <br /> SLS. The OSI audit includes; reviews of recycler compliance history, recycler worker <br /> safety and training, recycler closure plan and fund, recycler certificate of insurance, <br /> recycler laboratory tests of compliance sampling. Sylvania Lighting Services (SLS) <br /> cannot utilize lamp recyclers that are not approved by OSI. <br /> All removed lamps from your locations are brought back per State Universal Waste <br /> regulation to the branch facility and sorted into the appropriate containers. The amount of <br /> product in linear feet/units is noted on each SLS Service Service Control Form (SCF) as <br /> well as the location it was removed from. For your convenience we have supplied a <br /> recycling report that summarizes all recycled lamp activity. Non-universal waste items <br /> such as non-PCB ballasts or fixture housings may be disposed of properly on site <br /> depending on the local requirements for this waste. <br /> Your removed lamp material is consolidated with other removed lamps from other <br /> service customers. At specified time intervals a SLS approved recycler then removes all <br /> lamps for appropriate recycling at the branch. The branch will subsequently (as a result of <br /> recycling)receive a destruction certificate that summarizes the recycling activity in the <br /> linear feet/units processed.
