The following is an itemized list of hazardous waste violations that have not been <br /> addressed for E F KLUDT & SONS INC as of August 01, 2018. <br /> Open violations from February 26,2018 inspection <br /> Violation#109-Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manifest number 012478937JJK was not sent to DTSC within 30 days of shipment of <br /> hazardous waste. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when <br /> California is either the generator state or the destination or consignment state. The generator manifest copy shall <br /> be mailed to: <br /> DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copy of the manifests listed and ensure that copies of all generator manifests are sent to DTSC <br /> within 30 days of shipping hazardous waste. <br /> For the Household Hazardous Waste collection,there is only one manifest that is on the DTSC website for 2017 <br /> and none for 2015 or 2016. There were at least 20 other manifests for 2017 that were reviewed.
