Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 30, 2018 <br /> Facility Address: <br /> 345 Roth Rd, Lathrop <br /> Facility Name: <br /> Pilot Travel Center Lathrop - 1017 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 111 CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br />Manifest numbers 017497085JJK, 017497086JJK, 017497224JJK, 017497225JJK were found without a signed off copy <br />from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the <br />destination facility on site for three years and have them readily available for review. If the generator did not receive a <br />copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator’s <br />waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall <br />submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of <br />delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit <br />to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by April 29, 2018. <br />Documents reviewed by Regulator: Employee training log, disposal/manifest records. <br />Documents received by operator: Return to compliance certification form and inspection report. <br />Waste stream: fuel contaminated solids and fuel contaminated liquids. <br />Note: No modified contingency plan. An emergency coordinator and modified contingency plan information is <br />lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response <br />measures, and the following information must be posted by a phone: the name and phone number of the <br />emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency <br />coordinator and post the required information by a phone. A form was provided to Doug Humphreys during the <br />inspection to be used for this purpose. Doug Humphreys completed the modified contingency plan during the <br />inspection and this was corrected on site. <br />Note: Failed to completely label containers or portable tanks of hazardous waste. A black 55 gallon drum was <br />observed in the waste storage with a hazardous waste label containing solids and liquids lacking a hazardous <br />properties determination.
