Emergency Reporting <br /> Holz employees are not trained to combat any emergencies other <br /> than small fires and very small chemical spills. <br /> Whenever there is an imminent or actual emergency situation <br /> resulting from a fire, explosion, and/or known chemical release, it <br /> is the legal responsibility of the Emergency Coordinator (or <br /> designee) to notify the appropriate State and/or local agencies. <br /> An emergency notification phone roster is on page 3. <br /> If there is an emergency that could threaten human health or the <br /> environment outside the facility, the Emergency Coordinator must <br /> notify appropriate local authorities. He must also be available to <br /> help appropriate officials decide whether local areas should be <br /> evacuated and must notify the California Office of Emergency <br /> Services (800) 852-7550. <br /> Information to report: <br /> - Name and phone number of the person reporting. <br /> - Name and address of the facility. <br /> - Time and type of incident (fire, explosion, chemical <br /> release). <br /> - Name and quantity of material involved to extent known. <br /> - Extent of injuries, if any. <br /> - Possible hazards to human health or environment outside <br /> the facility. <br /> Within 15 days of any emergency incident which requires <br /> activation of the Contingency Plan, a written report of the <br /> incident must be submitted to the local agency, describing the <br /> incident.
