SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 600 East Main Street, Stockton, California 95202-3029 <br /> Telephone:(209) 468-3420 Fax:(209) 468-3433 Web:vwvw.sogov.org/ehd/unitiii.html <br /> CONTINUATION FORM Page: 3 of 5 <br /> OFFICIAL INSPECTION REPORT Date: 06/23/11 <br /> Facility Address: 8203 East Highway 26 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> San Joaquin County Environmental Health Department (EHD) has conducted a routine Hazardous <br /> Waste (HW) inspection of the referenced facility on June 23, 2011. The Official Inspection Report of this <br /> inspection was mailed on July 15, 2010. The facility is currently closed for business with no access to <br /> the building, but HW and petroleum fuel (in existing underground storage tanks) are still being stored on <br /> site. Following are all Class I violations observed during this inspection. <br /> #8,12,13,14,15,16,20,50,52,60,63, and #7 (this one is not a repeat) <br /> 7. Observed 2 bottles of used oil by garbage can between the dispensers. <br /> - Generator shall make hazardous waste determination of the waste and managing the waste <br /> accordingly. <br /> * Immediately make a hazardous waste determination of the above wastes and manage each <br /> accordingly, or dispose of the waste as hazardous waste in accordance to California Code of <br /> Regulations (CCR) Title 22. Submit how this violation was corrected and evidence to the EHD. <br /> 8. (Repeat) EPA number is inactive. <br /> -- Active EPA number is required to store or transport HW. <br /> * Immediately provide to EHD evidence of an active EPA number . <br /> 12. (Repeat) Unable to verify internal emergency communication for employee in response to an <br /> emergency. According to the contracted company Veeder Root (VR), VR is no longer providing services <br /> to the facility such as the spill control leader, as stated in the facility's Hazardous Materials Management <br /> Plan (HMMP). <br /> -- Facilities shall be equipped with an internal communications or alarm system capable of providing <br /> immediate instruction to facility personnel in response to an emergency. <br /> * Immediately provide to EHD evidence of communication or alert system available at the facility that <br /> can provide immediate instruction to personnel in response to an emergency. <br /> 13. (Repeat) Unable to verify provision on site for calling outside help in case of emergency. The <br /> facility is not manned, and VR has stated VR is no longer providing 24-hour monitoring as stated in the <br /> HMMP. The primary and alternate persons assigned to taking actions during an emergency on the <br /> HMMP could not be reached. <br /> -- Facilities shall be equipped with a device capable of summoning emergency assistance from the <br /> local police or fire departments, or State or local emergency response teams. <br /> * Immediately provide to EHD evidence of device for calling outside help available at the facility in <br /> case of an emergency. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> E /23-OF <br /> 3-0 Rev IO 10 CONTINUATION FORM <br />
