Unable to verify maintenance of spill control or decontamination equipment on site. <br /> --Facilities shall maintain spill control or decontamination equipment to ensure its proper operation in <br /> time of emergency. <br /> * Immediately provide to EHD evidence of proper maintenance or spill control or decontamination <br /> equipment at the facility. <br /> 16. Failed to provide access to communication or alarm during HW handling. <br /> -- Facilities shall be equipped with communication or alarm device at the facility immediate accessible <br /> to personnel handling HW. <br /> * Immediately provide to EHD evidence of communication or alarm device at the facility for personnel <br /> handling HW. <br /> 20. Observed one 55-gallon HW container behind the facility building (northeast corner) with no or <br /> illegible date of initial accumulation, content, physical state, and hazardous property information on the <br /> HW label. <br /> -- All HW containers must be labeled with the following minimum information: 1) Generator name <br /> and address, 3) Accumulation start date, 4) Content, 5) Physical state, 6) Hazardous Property <br /> * Immediately correct the violation as state above and provide evidence of correction to EHD. <br /> 50. Unable to verify the length of storage of the 55-gallon HW container. <br /> -- HW must be managed within 180 days of storage on Small Quantity HW generator facilities. <br /> * Immediately dispose the HW container to a properly permitted facility in accordance to Title 22, and <br /> submit a copy of the disposal receipt to EHD. <br /> 52. (Repeat) Failed to provide at least one employee responsible for emergency coordination. Unable <br /> to verify posting of emergency coordinator information and the modified contingency plan at the facility. <br /> Unable to verify training of personnel on HW handling and emergency procedures. <br /> -- Facilities shall provide at least one employee responsible on site or on call for emergency <br /> coordination. Facilities shall post by a phone the name and telephone number of the emergency <br /> coordinator, and the completed modified contingency plan.
