Eye wash bottles are found to be expired. The three bottles are the rear of the facility has an <br /> expiration date of 02/11 and the bottle at the front expired 01/14. All facility communications, alarm <br /> systems, fire protection equipment, spill control equipment, and decontamination equipment shall be <br /> tested and maintained as necessary to assure its proper operation in time of emergency. Immediately <br /> have all necessary decontamination equipment supplied for this facility, and ensure proper maintenance <br /> is conducted in the future. <br /> 116. Complete hazardous waste training has not been conducted for the employees on site in the last 12 <br /> months. Facility had recently installed a new SystemK4 dry cleaning machine within this past month to �e <br /> PERC/tetrachloroethylene, solvent . Last disposal receipt indicates waste PERC pick up of 12/30/13. <br /> New system was in used at time of inspection. Employees are not aware of the contents of its MSDS. <br /> Emergency response training and hazardous materials training must be offered annually. The training <br /> program shall be completed by each employee within six months of hire, prior to unsupervised work, and <br /> shall be reviewed annually. Immediately provide complete hazardous waste training to facility <br /> employees in accordance with Title 22 regulations. <br /> Submit to SJC EHD within 30 days 2/10/1 : <br /> 1. one copy of the completed 'Return o ompliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> 3. requested/ supporting documentation ie. signed manifests, training documents <br /> Notes: <br /> - Properly handle, store and dispose of used fluorescent tubes. Cannot be disposed of into garbage or <br /> be broken. Keep records of disposal receipts on site for a minimum of 3 years. <br /> - Contact Thuy Tran, Sr. REHS, ph. 468-8257 for assistance with CERS, California Electronic Reporting <br /> System for hazardous material, waste, inventory data submittals. CERS ID# 10183185 Information is <br /> incomplete. Must renew data annually. <br /> - Facility installed SystemK4, dry cleaning machine to replace PERC end of December 2013. SolvonK4 <br /> is halogen-free solvent, combustible, must use CO2/ powder fire extinguisher. CLIPK4 is detergent <br /> concentrate. Facility still generates hydrocarbon/ petroleum distillates/sludges from machines. <br /> ALL EHD STAFF TIE ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector.
