Turner Rd. Lodi, CA 95242 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> was broken and not in a closed container. A UW handler shall manage lamps in a way that prevents <br /> releases of any UW or component of a UW to the environment. A UW handler shall contain any lamp in <br /> a container or package that is structurally sound, adequate to prevent breakage, and compatible with the <br /> contents of the lamp. Such a container or package shall remain closed and shall lack evidence of <br /> leakage or damage that could cause leakage under reasonably foreseeable conditions. A UW handler <br /> shall immediately clean up and place in a container any lamp that is broken and shall place in a <br /> container any lamp that shows evidence of leakage or damage that could cause the release of mercury <br /> or other hazardous constituents to the environment. Containers shall be closed, structurally sound, <br /> compatible with the contents of the lamps and shall lack evidence of leakage, spillage, or damage that <br /> could cause leakage or releases of mercury or other hazardous constituents to the environment under <br /> reasonably foreseeable conditions. Correct immediately by managing UW lamps in a closed compatible <br /> container. Minor-Notice to Comply. <br /> 83. There are approximately thirteen boxes of UW lamps in the hazardous waste storage area. Some <br /> UW lamps were not labeled. A UW handler shall label or mark UW to identify the type of waste. Lamps <br /> (i.e., each lamp), or a container in which the lamps are contained, shall be labeled or marked clearly with <br /> the following phrase: "Universal Waste-Lamp(s)". Correct immediately by labeling the UW lamps as <br /> "Universal Waste Lamps". Minor-Notice to Comply. <br /> Note: The 5 gallon bucket of UW batteries were labeled as "waste batteries". A UW handler shall label or <br /> mark UW to identify the type of waste. Batteries (i.e., each battery), or a container in which batteries are <br /> contained, shall be labeled or marked clearly with the following phrase: "Universal Waste-Battery(ies)". <br /> Engineer wrote "universal waste batteries" on the 5 gallon bucket of UW batteries during the inspection. <br /> 84. There are approximately thirteen boxes of UW lamps in the hazardous waste storage area. Some <br /> UW lamps were not in boxes. The facility did not demonstrate when the UW lamps became a UW. A UW <br /> handler shall accumulate UW for no longer than one year from the date the UW was generated, or was <br /> received from another UW handler. A UW handler shall be able to demonstrate the length of time that <br /> the UW has been accumulated from the date it became a waste or was received. The UW handler may <br /> make this demonstration by: <br /> (1) Placing the UW in a container and marking or labeling the container with the earliest date that any <br /> UW in the container became a waste or was received. <br /> (2) Marking or labeling the individual item of UW with the date it became a waste or was received. <br /> (3) Maintaining an inventory system on site that identifies the date the UW being accumulated became a <br /> waste or was received. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspedo Received By: t3 'yea Title: <br /> k diw I. <br /> EHD23-02-003 Rev00/10/10 CONTINUATION FORM <br />
