This notice was issued following a 5/04/2012 <br /> inspection at our 4545 Qantas Lane facility. The findings and corrective actions are as <br /> follows: <br /> #121 Failed to send generator manifest copies to DTSC within 30 days. <br /> Manifest numbers 000445587VES (6/24/11), 000543018VES (11/l/11). <br /> 000543348VES (2/23/12), 000543382VES (2/23/12) and 000543499VES <br /> 4/24/42)were submitted to DTSC (See enclosed) <br /> #122 & 705 Failed to keep signed copies of manifests for 3 years and Failed to send <br /> copy of generator manifest to DTSC with an indication that copy of manifest signed <br /> by destination facility was not received after 60 days. <br /> • Signed copies of manifest numbers 000544565VES (9/7811), 000543348VES <br /> (2/23/12) and 000543499VES (2/23/12)were located and submitted to DTSC (see <br /> enclosed— same as above) <br /> Plant personnel have been trained on the requirements for manifest preparation; filing and <br /> maintenance to ensure all manifests are appropriately generated and managed going <br /> forward. Also enclosed are pictures documenting the following items noted as"corrected <br /> on site": <br /> • Modified contingency plan completed and posted. <br /> • Used fluorescent tubes placed in a closed container. <br /> • Used fluorescent tubes labeled. <br />
