By June 4, 2012, submit to the EHD a copy of the signed off copy of the manifests or a copy of <br /> the information submitted to DTSC. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by June 4, 2012 along <br /> with a statement documenting the corrective actions that have been or will be taken for each violation <br /> and any supporting paperwork. <br /> Notes: (corrected on site) <br /> - Modified contingency plan information was lacking. The following information must be posted by a <br /> phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill <br /> control equipment, and if present, fire alarm; and the phone number of the fire department, unless the <br /> facility has a direct alarm. A modified contingency plan was completed and posted at the time of <br /> inspection. <br /> - Used fluorescent tubes were observed stored open and uncontainerized in the electrical room. <br /> Universal waste lamps shall be contained in a closed container or package that is structurally sound, <br /> adequate to prevent breakage, and compatible with the contents of the lamp. The tubes were placed in <br /> a closed container at the time of inspection. <br /> - Used fluorescent tubes in the electrical room and a container of electronic waste outside the hazardous <br /> waste storage area were observed without universal waste labeling. A generator shall label or mark <br /> universal waste with the words 'Universal Waste' and the type of waste (batteries, mercury containing <br /> equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol cans). These wastes were <br /> labeled at the time of inspection. <br /> Note: The facility changed ownership in April 2011. <br /> Provided: Return to Compliance Certification form, Modified Contingency Plan template, Schedule of <br /> FREE classes <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES KOLL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
