TFD reported the incident to Garrett Backus of the San Joaquin County Environmental Health <br /> Department (EHD) on October 19 at 12:45 am. <br /> OBSERVATIONS/COMMENTS: <br /> Garrett Backus of the EHD discussed the incident by telephone with Steve Hanlon (209) 640-4337 <br /> and Taylor Farms representative Antonio Zapien (209) 321-3859 at 1 am on October 19. Antonio <br /> said the lettuce cleaning is done with a chlorine solution mixed with T-128-smartwash. Antonio said <br /> the cleaning solution was out-of-balance, possibly causing the worker injuries. He said the solution <br /> was titrated to bring back into balance and is working correctly at the time of my telephone call. He <br /> said two bins of lettuce of approximately 500 pounds each were set aside for evaluation. <br /> CORRECTIVE ACTIONS: <br /> Garrett Backus of the EHD referred the worker injuries to Cal-OSHA, Modesto District Office, (209) <br /> 545-7310, by telephone and in writing. <br /> Garrett Backus of the EHD referred the two bins of lettuce set aside for evaluation to the US FDA <br /> Consumer Complaint Coordinator, (510) 337-6741, by telephone. <br /> CORRECT BY: NIA <br /> INSPECTED BY: <br /> RECEIVED BY: 7 / <br /> �ce� 'f r� DATE: <br /> EHD 48-05 Rev.04/19/12 INSPECTION REPORT <br />
