6. Hazardous Waste Program Inspection Minor Violation#15 Summary and
Response-
a. The spill pallet located in the Oil Storage Room was observed with
standing dark liquid in the bottom. All facility communication and alarm
systems, fire protection equipment, spill control equipment, and
decontamination equipment, shall be tested and maintained as necessary to
assure its proper operation in times of emergency. Spill pallets shall be
maintained clean and dry at all times so as to catch any unintentional
release from the primary containment. Immediately remove and properly
dispose of all hazardous waste from spill pallets, and maintain all spill
containment clean and dry at all times.
i. Response- Spill pallets were emptied and cleaned. The contents
were disposed in the appropriate waste stream. The Oil Storage
Room is fully contained in secondary containment making the spill
pallets redundant. Spill pallets will be used elsewhere in
operations.
ii. Our Standard Operating Procedure for Spill Response is edited to
include immediate clean-up of any impacted secondary
containment.
iii. Further, our corporate weekly multi-media checklist (FO 1-B) is
locally edited to provide a routine check on all secondary
containment of the facility to trigger a corrective or preventive
action.
b. STATUS: Complete
c. Attached documents
i. "Before" pictures, spill pallets in Oil Storage.
ii. "After" pictures, spill pallets cleaned and removed from secondary
containment.
iii. Updated Spill Response SOP, with highlighted change on page 3
of the SOP.
iv. Updated Area Environmental Inspection Form-Manufacturing,
Form FO 1-B with the changed Item 8. Highlighted.
