Original .port by Mr. Sooft states on 9/29/2016 Q 0950 hours one ruptured lamp on a UV reactor, one bulb equals 2.3 grams of mercury. Cleaned and did not Impact. <br />@1500 hours EHO Lisa M. contacted Antrany Sinn for phone interview. Per Mr. Scott the facility is a primary real facility and has 12 UV reactors. Upon staff conducting a routine calibration a <br />lamp broke releasing up t0 2.3 grams of mercury. Response teams took ore of Ma spill, no employees wereaffected. There was an In house mercury response according. SOPs and initial <br />w48cabons continued and operations were shut down. Per Mc Scott visual observation of the released mercury was contained within Me UV reactor room. <br />Approach to clears -up after physical mmoval of mercury was a wash down of Me areas) Into drain leading to an on-site will pond. <br />Per Mr. Scott after assessing safe approach to Me m3d0glesting with mercury measuring devices was concluded and a series of water samples were taken from drains leading into pond wiM <br />spi protocols at compliance points Per Mr Scott no memory was battered at compliance points Mail East located ,n Sunol off Calaveras Road.. <br />Facility was directed to report release to CALOES on 9/30/2016 for 9/27/2016 release. <br />Facility will respond to the EHD with required documents on 10/3/2016. <br />Revised 6101/2015 Page 1 of 2 NOTIFICATION OF HAZ WASTE DISCHARGE <br />
