6 <br /> The following is an itemized list of hazardous waste violations that have not been <br /> addressed for WEST LANE MUFFLER & PERFORMANCE as of December24, <br /> 2015. <br /> Open violations from November 06, 2014 inspection <br /> Violation#101 -Managed a hazardous waste without a valid EPA ID number. <br /> This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or <br /> offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and reactivate your <br /> EPA ID number and submit evidence to the EHD. A form has been provided. This is a minor violation. <br /> Violation#109 -Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for November 2011 through November 2014 were r <br /> found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and <br /> all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for <br /> review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and <br /> submit copies to the EHD.This is a Class II violation. <br /> Violation#112 -Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for November 2011 through November 2014 were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts use <br /> in a consolidated manifesting procedure on site for three years and have them readily available for review. Immedial <br /> locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br /> EHD. This is a minor violation. <br /> Violation#302 -Failed to maintain required emergency equipment. <br /> Spill control equipment was not found on site. All facilities shall be equipped with an internal communication or alarrr <br /> system, a device capable of calling outside emergency help, spill control equipment, and decontamination equipmen <br /> at all times. Immediately provide spill control equipment and maintain on site. This is a minor violation. <br /> Violation#608-Failed to properly manage used oil and fuel filters. <br /> A black 55 gallon drum of used drained oil filters behind the shop was stored on site for longer than one year. <br /> Facilities that generate up to one ton of used oil filters per year may store the filters up to one year. Facilities that <br /> generate one ton or more of used oil filters may store the filters for up to 180 days. Immediately recycle these filters <br /> under a bill of lading and submit a copy of the bill of lading to the EHD. This is a Class II violation. <br /> A black 55 gallon drum of used drained oil filters behind the shop was open during time of inspection . Used oil filten <br /> shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as"drained used oil filters,"and <br /> marked with an accumulation start date. Immediately store the drained used oil filters in a closed container and <br /> ensure that all used oil filters are managed in this way. This is a Class II violation. <br /> Paae 1 of 1 <br />
