On several checklist observations were written with no <br /> corrective action to address. Weekly inspections should be used as a tool to identify and correct deficiencies. <br /> 406 CCR 66261.7 Contaminated container not marked with date emptied or managed within one year of being emptied. <br /> Empty blue poly drum, 30 gallons, that previously held formalin was seen in the bunker with a hazardous waste <br /> label (photo 98).Containers larger than five gallons that previously held a hazardous material shall be marked with <br /> the date they were emptied and managed within one year of being emptied. Immediately mark and manage all <br /> empty containers per this section. <br /> This is a Class II violation. <br /> 601 CCR 66262.34(a) Stored hazardous waste on site longer than 90 days without a permit or authorization. <br /> Black hazardous waste container of empty Warfarin containers, in the bunker, had a start accumulation date of <br /> 3/14/17, indicating storage of HW at 91 days (photo 95) White hazardous waste bucket of aerosols, in the bunker, <br /> had a start accumulation date of 2/8/17 indicating storage of HW at 125 days (photo 83) . Facilities who generate <br /> more than 1000 kg of hazardous waste per month may store waste on site up to 90 days. Immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the <br /> EHD. <br /> This is a Class I violation. <br /> 602 CCR 66262.34(e)(1)(B) Stored satellite accumulated waste on site longer than one year or 90 days after fill date. <br /> 1 white hazardous waste bucket seen in Family Medicine 1, storing silver nitrate, was seen with two start <br /> accumulation dates, 4/26/16 and 2/4/2016, both over the 1 year allowable storage time for satellite containers. <br /> Satellite accumulated waste shall not be stored on site for more than 90 days after the container was filled, or a total <br /> of one year, whichever comes first.
