Caused transportation of hazardous waste to an unauthorized point. Holt of California performed <br /> preventive maintenance service on a diesel fueled generator set at this facility. On 12-18-10, 88 gallons <br /> of used motor oil was transported from this address to Holt of California's facility for consolidation. <br /> Pursuant to California Health & Safety Code (HSC) 25250.23, transportation of used oil requires a <br /> hazardous waste (HW) transporter registration. <br /> HSC 25250.12 allows a mobile maintenance operator, without HW transporter registration, to transport <br /> not more than 55 gallons of used oil in the vehicle at one time. On 12-18-10, Holt transported 88 gallons <br /> of used oil and did not qualify for the HSC 25250.12 exemption. <br /> Send a statement to the Environmental Health Department (EHD) by May 12, 2011 explaining how the <br /> facility will come back into compliance. <br /> Notes <br /> 1. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by May 12, 2011. <br /> 2. The facility generated 1.7 tons of hazardous waste in 2010. <br /> 3. Incomplete hazardous waste labels were completed during the inspection in the clinical laboratory, <br /> dermatology, and the inpatient pharmacy.
