SAFETY MANAGEMENT PROGRAM POLICY/PROCEDURE MANUAL <br /> • TITLE: HAZARDOUS WASTE CONTINGENCY PLAN POLICY NUMBER 5.06 <br /> PAGE 11 OF 14 <br /> generation, or ruptures in valves, pipes, or other equipment, wherever this <br /> is appropriate. <br /> 3.9.15 Immediately after an emergency, the emergency coordinator shall provide <br /> for treating, storing, or disposing of recovered waste, contaminated soil or <br /> surface water, or any other material that results from a release, fire, or <br /> explosion at the facility. If the recovered material is hazardous, it shall be <br /> handled according to all policy and procedures for hazardous material. <br /> 3.9.16 The emergency coordinator shall ensure that, in the affected area(s) of the <br /> facility: <br /> 3.9.16.1 No waste that may be incompatible with the released material <br /> is transferred, treated, stored, or disposed of until cleanup <br /> procedures are completed; and <br /> 3.9.16.2 All emergency equipment is cleaned and fit for its intended use <br /> before operations are resumed. — <br /> 3.9.16.3 The administrator (or designee) shall notify the Department of <br /> Health Services, and appropriate State and local authorities, that <br /> • facility is in compliance with the above noted section before <br /> operations are resumed in the affected area(s) of the facility. <br /> 3.9.16.4 The administrator (or designee) shall document the time, date, <br /> and details of any incident that requires implementing the <br /> contingency plan. Within 15 days after the incident, the <br /> administrator (or designee) shall submit a written report on the <br /> incident to the Department of Health Services. The report shall <br /> include: <br /> • Name, address, and telephone number of the administrator <br /> (or designee); <br /> • Name, address, and telephone number of the facility; <br /> • Date, time, and type of incident (e.g., fire, explosion); <br /> • Name and quantity of material(s) involved; <br /> • The extent of injuries, if any; <br /> • An assessment of actual or potential hazards to human <br /> health or the environment, where this is applicable; and <br /> • Estimated quantity and disposition of recovered material <br /> that resulted from the incident. <br /> 3.9.16.5 Emergency Phone Numbers: <br /> • Fire Department 911 <br /> • • Police Department 911 <br /> • Administration Pager 546 -8718 <br /> Fresno Revision dates: 09/12/96, 6/25/98, 3/30/00, S/1/0 1,Stockton revision date 07/22/02, <br /> 09/19/02, 3/20/03 <br />
