SAFETY MANAGEMENT�ROG <br /> RAM POLICY/PROCEDURE ALNUAL <br /> TITLE: RAZAntious WASTE CoNTMENCY PLAN POLICY NUMBER 5.06 <br /> PAGE 10 OF 13 <br /> 3.9.11 The evacuation should take place to the department's designated assembly area <br /> (see each departmental plan) unless advised otherwise (see attachment A, <br /> general facility evacuation route) <br /> 3.9.12 The emergency coordinator shall, in every situation, immediately notify the <br /> State Office of Emergency Services. The report shall include: <br /> • Name and telephone number of reporter; <br /> • Name and address of facility; <br /> • Time and date of incident(e.g.,release,fire); <br /> • Name and quantity of material(s)involved,to the extent known; <br /> • The extent of injuries, if any;and <br /> • The possible hazards to human health, or the environment, outside the <br /> facility. <br /> 39.13 During an emergency, the emergency coordinator shall take all reasonable <br /> measures necessary to ensure that fires, explosions, and releases do not occur, <br /> recur, or spread to other hazardous waste at the facility. These measures shall <br /> include, where applicable, stopping processes and operations, collecting and <br /> containing released waste,and removing or isolating containers. <br /> 3.9.14 If the facility stops operations in response to a fire, explosion or release, the <br /> emergency coordinator shall monitor for leaks, pressure buildup, gas <br /> generation, or ruptures in valves, pipes, or other equipment, wherever this is <br /> appropriate. <br /> 3.9.15 Immediately after an emergency, the emergency coordinator shall provide for <br /> treating, storing, or disposing of recovered waste, contaminated soil or surface <br /> water, or any other material that results from a release, fire, or explosion at the <br /> facility. If the recovered material is hazardous, it shall be handled according to <br /> all policy and procedures for hazardous material. <br /> 3.9.16 The emergency coordinator shall ensure that, in the affected area(s) of the <br /> facility: <br /> 3.9.16.1 No waste that may be incompatible with the released material is <br /> transferred, treated, stored, or disposed of until cleanup procedures are <br /> completed;and <br /> 3.9.16.2 All emergency equipment is cleaned and fit for its intended use <br /> before operations are resumed. <br /> 3.9.16.3 The administrator (or designee) shall notify the Department of <br /> Health Services, and appropriate State and local authorities,that facility <br /> is in compliance with the above noted section before operations are <br /> resumed in the affected area(s) of the facility. <br /> 3.9.16.4 The administrator (or designee) shall document the time, date, and <br /> details of any incident that requires implementing the contingency plan. <br /> Within 15 days after the incident, the administrator (or designee) shall <br /> Fresno Revision dates: 09/12/96, 6/25/98,3/30/00, 8/l/01,Stocklon revision date 07122/02, <br /> 09/19/02 <br />
