• i <br /> SAFETY MANAGEMENT PROGRAM POLICY/PROCEDURE MANUAL <br /> TITLE: HAZARDOUS MATERIAL SPILL MANAGEMENT POLICY NUMBER 5.06 <br /> PAGE 5 OF 9 <br /> 3.2.5.4 Upon satisfactory clean up of the spill, the Safety Director will <br /> notify appropriate regulatory agencies, if required. <br /> 3.2.5.4.1 A written report will be provided to the Operations <br /> Support Leader, within 24 hours, which details the <br /> response and follow up required. <br /> 3.2.6 External Agency Support <br /> 3.2.6.1 The contracted external agency is activated if; <br /> 1) The spill is of a volume that meets criteria for an <br /> emergency (major) spill see attachment). <br /> 2) The spill amount may violate or has violated <br /> personal exposure limits, short-term exposure limits <br /> or ceiling limits. <br /> 3) Available department staff have not received <br /> Incidental Hazardous Waste Spill Training for <br /> "first response" at the awareness level. <br /> 3.2.6.2 If activated with the approval of the Safety Director (or <br /> designee) and upon arrival at the scene, the External Spill <br /> Response Team will provide assistance at the direction of <br /> the Safety Director (designee) and will assume the role of <br /> Incident Commander. <br /> •The Hazardous Materials Spill Team Incident <br /> Commander will coordinate the response as soon as <br /> possible. <br /> 3.2.6.3 Engineering staff member(s) will be notified if necessary to <br /> maintain that ignition sources are shut off if the material <br /> involved is flammable. <br /> 3.2.6.4 Engineering staff member (s) will work with the Incident <br /> Commander to determine if ventilation systems need to be <br /> increased or partially/completely shut down and will <br /> proceed appropriately. <br /> Fresno Revision dates: 09/12196, 6125/98, 3130/00, 811/01,Stockton revision date 07/22102 <br />
