Wilson Way, Stockton Program: HW <br /> SU Y OF VIOLATIONS <br /> CLASS I, LASS ,or MINOR-Notice to Comply) <br /> This is a Routine hazardous waste inspection. The following are repeat Class II violations: 8, 52 <br /> 8. (Repeated violation) This facility's EPA ID number is inactive. A hazardous waste generator shall not <br /> treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. <br /> Immediately contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by July <br /> 15, 2011. A form has been provided. <br /> 52. (Repeated violation) An emergency coordinator and modified contingency plan information is <br /> lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency <br /> response measures, and the following information must be posted by a phone: the name and phone <br /> number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if <br /> present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the required information by a phone. A form is <br /> provided that can be used for this purpose. Submit evidence of correction to the EHD by July 15, 2011. <br /> NOTES <br /> The facility generated no hazardous waste or universal waste the last three years according to manager. <br /> Discussed with manager Universal Waste management requirements including aerosol cans, lamps, <br /> alkaline batteries, mercury containing switches, CRT, electronic devices, etc. in a closed container with a <br /> label identifying the content and the start date of accumulation. Universal Wastes must be managed <br /> within 1 year of accumulation, and the disposal records maintained on site for at least 3 years. <br /> A copy of Retail Gasoline Outlets Management Practice factsheet, CUPA class schedule, blank <br /> hazardous waste labels, hazardous waste labeling factsheet, and San Joaquin County Small Business <br /> Hazardous Waste Disposal Program pamphlet, have been provided to generator during this inspection. <br /> Submit the following to EHD by: July 15, 2011 <br /> 1) A completed copy of the (provided) Return to Compliance Certification (please keep original for <br /> your records). <br /> 2) A written statement how each violation was corrected <br /> 3) Evidence in support of the corrections made. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THI FACILITY IS SUBJECT TO REINSPECTION AT ANYTIME AT EHD'S CURRENT HOURLY RATE. <br /> EH n ector Received By: u W""` � Title: <br /> � 14 <br /> D 23- 0 8110/10 CONTINUATION FORM <br />
