The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JOHN JESSUPS DREAM RIDES as of December31, 2015. <br /> Open violations from July 08,2015 inspection <br /> Violation#102-Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br /> years. <br /> 3-55 gallon drums of part washer solvent. Any person who generates a waste shall determine if the waste is a <br /> hazardous waste. Immediately make a hazardous waste determination for the 3-55 gallon drums of part washer <br /> solvent and manage it according the Title 22 hazardous waste regulations. Submit a statement and supporting <br /> documentation explaining how this waste was managed. <br /> Violation#109-Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for used oil for 2015 were not found on site. Accord <br /> to John, the used oil was picked up a week before the inspection date, July 8. Hazardous waste generators shall <br /> retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting <br /> procedure on site for three years and have them readily available for review. Immediately locate a copy of all manife <br /> and receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> Violation #112-Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for used oil for 2015 were not found on site. According to John,the user <br /> oil was picked up a week prior to the inspection date, July 8. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for <br /> three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for th <br /> last three years, maintain them on site, and submit copies to the EHD. <br /> Violation #117-Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at hftp://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Page 1 of 1 <br />
