Some financial responsibility documentation was received by the EHD on 9-5-2014, however, a <br /> complete set of financial responsibility documents has not been submitted to the EHD. Specifically, a <br /> current copy of the Letter from Chief Financial Officer (CFO) was not submitted to the EHD. A blank form <br /> was provided at the time of inspection and an updated copy of the Letter from CFO was provided to the <br /> EHD at the time of inspection. In the future, you may log into CERS to upload the required information, <br /> and submit for review by the EHD. CORRECTED. <br /> 302 & 304. The L4 annular sensor failed to activate an audible and visual alarm when tested and, <br /> therefore, the tank is not monitored. The interstitial space shall be monitored with a monitoring system <br /> connected to an audible and visual alarm system. The technician onsite cleaned the sensor and <br /> retested with passing results. CORRECTED. <br /> 310. Approximately 1-3 cups of liquid was observed in the diesel STP sump. If water could enter into <br /> the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> The technician onsite removed all the liquid from the aforementioned sump and retained the removed <br /> liquid for testing purposes. CORRECTED. <br /> A copy of the Return to Compliance Certification form was submitted to the EHD at the time of <br /> inspection. <br /> Note: Facility operators are required to upload the following UST program documents into the <br /> California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of <br /> Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), <br /> UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br /> Compliance.
