The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). According to <br /> the service requestor, no piping modifications will take place during the proposed <br /> conversion frames installation. <br /> a. Verification of conversion frames installation per the manufacturer's installation <br /> instructions <br /> b. Verification of the dispenser footprints contained by the under dispenser containment <br /> sumps (UDCs) and verification that all potential product piping leaks will be captured <br /> in the UDC: <br /> c. Visual inspection of the piping within the dispensers for any product leaks <br /> Please be aware that the facility cannot operate the UST system prior to the EFiD <br /> inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
