Onsite personnel must initially complete a 40-hour hazardous materials training course as <br /> required by Code of Federal Regulations (CFR) 1910.120. Thereafter, personnel are <br /> required to annually complete an 8-hour hazardous materials refresher course. Use of <br /> respirators must be in accordance with the written respiratory protection program. Personnel <br /> must be properly trained and fit-tested for the respirator worn. <br /> 11.0 MEDICAL SURVEILLANCE <br /> According to CFR 29, 1910.120, Paragraph (f), employees who wear respirators 30 days or <br /> more during I year or who have been exposed to hazardous substances or health hazards <br /> above established PELs are required to be medically monitored. While airborne <br /> contamination levels are anticipated below permissible PELs, respirators fitted with organic <br /> vapor cartridges should be worn whenever the smell of gasoline is present. Consequently, <br /> onsite personal must participate in a medical surveillance program. <br /> 12.0 RECORD KEEPING <br /> Documentation will be kept on all personnel exposed to contaminant hazards on the job site <br /> according to OSHA regulations. This will include documentation that employees have <br /> received training on the SSP, respiratory protection, MSDS forms and all emergency <br /> procedures. These will be reviewed during the pre-site training meeting. <br /> Exposure records on each job will be kept for 30 years to meet regulatory requirements. <br /> Included will be names and Social Security numbers of employees, medical evaluations, on- <br /> the job jogs from entry to exit, first aid administered, visits onsite by non-employees and <br /> personal air monitoring records. <br /> 13.0 EMERGENCY RESPONSE AND CONTINGENCY PLAN <br /> In the event of accident, injury, fire, explosion or other emergency, the Project Geologist, <br /> SSO, or designated representative will be responsible for coordinating emergency response <br /> activities. The SSO or representative will call 911 and contact the hospital. <br /> The SSO will contact any appropriate government agencies. During an emergency situation <br /> the following steps will he implemented: <br /> 1. The SSO will verbally notify onsite personnel of the emergency and direct <br /> personnel. <br /> 2. If the emergency cannot be readily contained, extinguished, or controlled by <br /> onsite personnel, the SSO will call 911 and inform them of the location and <br /> details of the emergency situation. <br /> 3. The SSO will notify the Project Manager and Principal, if necessary. <br /> 4. The SSO, with the assistance of the Project Manager, will decide when to resume <br /> operations after an incident has been controlled. <br />
