Weber Avenue, Third Floor Stockton, California 95201-0388 Re: UST Permit Application Forms A and B Former Unocal Service Station 0123 1034 Central Avenue at 11th Street, Tracy, California Dear Mr. Infurna: Enclosed are the completed underground storage tank (UST) permit application forms A and B for closure of the two USTs at the above referenced site. Also enclosed is a revised UST program fee worksheet and a check for$280. This check and our previously submitted check in the amount of$4,492 will cover the required UST registration and penalty fees, as determined by the revised fee worksheet. It is my understanding that the enclosed forms and check will complete our tank closure application which was originally submitted on April 12, 1996. As we discussed during our telephone conversation on May 2, 1996, final approval of the UST closure in place application is pending a written response from the City of Tracy Department of Public Works, which will provide their position regarding the allowance of sidewalk excavation to remove the USTs versus closure in place.
