Health Department (EHD) has completed the review of the <br /> application for a permit to seal a Diversified Product Manufacturing penetration using Diversified Bulkhead <br /> Bonder on the diesel#2 submersible turbine pump (STP) sump on the underground storage tank(UST) <br /> system at the above noted address. This permit has been amended from its original version and <br /> supersedes the permit issued on the August 29, 2017. This permit was amended to include the repair of <br /> the outside sheeting of the 100 octane Veeder-Root annular 407 sensor which was conducted on June <br /> 27, 2017.The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Diversified Bulkhead Bonder may only be used on Diversified penetrations only after being <br /> properly cleaned with Diversified Bulkhead Cleaner, DBC II, per manufacturer specifications. <br /> Work on penetrations which are not manufactured by Diversified Product Manufacturing will <br /> require an amended scope of work. <br /> 4. Contact Cesar Ruvalcaba at(209) 953-6213 to arrange the following appointments (a 48-hour notice <br /> is required for inspection appointments). <br /> • A 24 hour hydrostatic secondary containment test, or its equivalent, on the diesel #2 STP <br /> sump. (This testing has been satisfied.) <br /> • Sensor functionality:Functionality of the 100 octane annular sensor,must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California Code of <br /> Regulations and the Health&Safety Code. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br /> and approval. <br /> 5. If concrete will be broken near the diesel#2 tank, a secondary containment test must be performed on <br /> the diesel#2 tank once concrete is poured in accordance with the manufacturer's guidelines. <br /> 6. Secondary containment testing for the impacted sump is required six months after installation or repair. <br /> Provide 48 hours notification to the EHD when this test is scheduled. <br /> 7. Any individual performing the work of a service technician must meet the following qualifications: <br />
