Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Cesar Ruvalcaba at(209)953-6213 to arrange the following appointments(a 48-hour notice <br /> is required for inspection appointments). <br /> • Verification of the dispenser footprints contained by the under dispenser containment sumps <br /> (UDCs) <br /> • Visual inspection of the piping within the dispensers. <br /> • Verification of the installation of the Bravo CONV-62000 conversion frame per manufacturer <br /> instructions and the Bravo Dispenser Conversion Evaluation Form shall be onsite at the time <br /> of inspection. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br /> and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be performed on the tank <br /> once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, current <br /> contractor's licenses issued by the Contractors State License Board (may be issued to the <br /> installation company), or a Tank Tester's license issued to the individual by the State Water <br /> Board,and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at <br /> the work site who possesses a valid,current California UST Service Technician certificate issued <br /> by the International Code Council(ICC),and <br /> c. Possesses a valid,current certificate of training from the manufacturer(s)of the component(s) <br /> being tested, repaired,or serviced. <br />
