The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Secondary containment testing on UDC 11/12, 8/9(sat), 9/10 and 14(sat) sumps. <br /> b. If concrete will be broken near the UDC 11/12, 8/9(sat), 9/10 and 14(sat) sumps then <br /> a secondary containment test must be performed on the product piping once <br /> concrete is poured in accordance with the manufacturer's guidelines . <br /> 4. Secondary containment testing for the UDC 11/12, 8/9(sat), 9/10 and 14(sat) sumps is <br /> required six months after installation. Provide 48 hours notification to the EHD when this <br /> test is scheduled. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br /> (may be issued to the installation company), or a Tank Tester's license issued to the <br /> individual by the State Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service <br /> Technician certificate issued by the International Code Council (ICC), and <br />
