Northwestern Equipment & Supply Company located at 2245 W. Charter <br /> Way. The facility is owned by Mr Doc Raymond of California Welding <br /> Supply, 817 S. Center St. Phone # 209-466-8604. <br /> 2. A plot plan is supplied with this letter in duplicate. <br /> 3. The work will be performed by SEIBOL.D CORPORATION. License # 472610 <br /> A, B, and C-10 classifications. A Workman 's Comp Cirtificate is <br /> submitted with this letter. <br /> 4. We are removing two 10,000 gallon underground tanks that previously <br /> supplied unleaded and diesel fuel. <br /> 5. J.H. Kleinfelter & Associates, 2825 Myrtle Ave of Stockton will <br /> perform required soil samples and a release of information <br /> authorization form is hereby submitted. <br /> 6. The tanks are expected to be totally empty of any fuel or residue. <br /> If any residual liquid, solids or sludges are encountered, we will <br /> place it into a 5 gallon container and given to the owner if useable <br /> or taken to a waste disposal dump if considered waste by state <br /> standards. I <br /> 7.
