U-Haul <br /> Facility Number 709-83,450 North Cherokee Lane <br /> Lodi, California <br /> WPI Project#96237 <br /> HEALTH AND SAFETY PLAN <br /> Page 5 <br /> SECTION 3.0 <br /> 3.1 MEDICAL SURVEILLANCE <br /> WPI personnel engaged in project operations will be participants in a Medical Surveillance <br /> Program, and must be cleared by the examining physician to wear respiratory protection devices <br /> and protective clothing for working with hazardous materials. The requirements under California <br /> Administrative Code,Title 8, Section 5216 will be observed. <br /> 3.2 SUBCONTRACTOR MEDICAL SURVEILLANCE <br /> Medical surveillance for subcontractor personnel will be consistent with the requirements of <br /> Section 3.1 above. <br /> 3.3 EMERGENCY FACILITIES <br /> As appropriate, contacts will be established with emergency facilities and local agencies, <br /> including local hospital facilities, law enforcement agencies, local fire departments, and <br /> ambulance services prior to starting work. The Fire or Police Departments can be contacted by <br /> dialing 911. Directions to Lodi Memorial Hospital, 975 South Fairmont Avenue, Lodi, <br /> California are included in Section 7.2.5. <br /> SECTION 4.0 <br /> 4.1 EMERGENCY PLANNING <br /> To fully meet the needs of the Health and Safety Plan, WPI will address the emergency planning <br /> needs this site. This may include communication with the owner/operator, evaluation of the spill <br /> response measures, and interaction with the local response community and neighboring property <br /> owners. Residual gasoline and diesel have been removed by U-Haul, thereby removing the <br /> possibility of any significant spillage. Triple rinsing performed prior to removal of the tanks will <br /> also preclude the possibility of product release. The emergency assembly areas are identified on <br /> Figure 2. <br />
