3.0 UNDERGROUND STORAGE TANK CLOSURE PROGRAM <br /> The underground storage tank closure program involved a five step process including: (1) Tank Closure <br /> Permitting, (2)Tank System Removal and Disposal, (3) Soil Sampling, (4) Laboratory Analysis, and(5) <br /> Backfilling and Resurfacing the Excavation. Each element is described in the following sections. <br /> 3.1 TANK CLOSURE PERbIITTING <br /> On December 17, 1996,Wayne Perry,Inc.Obtained an Underground Storage Tank Permit#29959 from the <br /> City of Lodi. On December 17,1996,Wayne Perry,Inc.also obtained a tank removal permit from the Lodi <br /> Fire Department. Copies of the closure permits are included in Appendix A. <br /> 3.2 TANK SYSTEM REMOVAL AND DISPOS i <br /> On December 17, 1996, Wayne Perry, Inc., utilized a John Deere (Model 31OD) backhoe to remove the <br /> concrete and partial asphalt surface cover and dispenser island and exposed the top of the underground <br /> storage tanks. During the excavation, the soil removed from the excavations(and temporarily stockpiled <br /> near the excavation)was monitored using a photoionization detector. <br /> On December 18, 1996, Adams Services of Gardena, California, under subcontract to Wayne Perry, Inc., <br /> rinsed and purged the underground tanks in accordance with applicable State and local regulations. A copy <br /> of the rinse/purge documentation and transportation manifest are presented in Appendix B. <br /> On December 18, 1996,prior to removal of the tank,a representative from the City of Lodi and the Lodi fire <br /> Department monitored the Lower Explosive Limit (LEL) levels in the tanks and approved the removal. <br /> Following the approval to proceed,Adams Services removed the tanks using a crane. The tanks were loaded <br /> onto a transport trailer, properly secured, and transported to Adams Steel facility located in Anaheim, <br /> California where the tanks were properly destroyed. The certificate of tank destruction is included in <br /> Appendix C. <br /> Concurrent with the tank removal program, Wayne Perry, Inc. removed the product dispenser, dispenser <br /> island, and vertical vent piping from the site. The dispensers, vertical vent piping, and miscellaneous <br /> concrete and asphalt debris were transported to a landfill for disposal. <br /> 3.3 SOIL SAMPLING <br /> On November 22, 1996, a representative of Blaes Environmental collected soil samples from the tank <br /> 4 <br />
