Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class II. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The response plan is not current and not uploaded to the California Environmental Reporting System (CERS).The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document the correct facility name from March to July 2014 on the designated operator monthly inspection reports. The facility name was listed as Oasis Plaza. It should have been listed as Plaza Liquor#1. Ensure that designated operators performing monthly inspections at this facility are including all of the correct information on the reports. This is a repeat violation, Class II. 314 CCR 2631(a) Primary containment not tight(pre-Jul 2003). An active leak was observed at the diesel filter at dispenser 3/4. A drop of fuel was observed at the exterior bottom of the 91 filter at dispenser 3/4. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a minor violation.
