The map does not include the locations of piping. A site <br /> plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the <br /> California Environmental Reporting System (CERS) at http-.Hcers.calepa.ca.gov/and upload a copy of the UST <br /> Monitoring Site Plan. <br /> This is a minor violation. <br /> 2,06 CCR 2712 Failure to comply with UST operating g permit conditions. <br /> The work orders on site indicate that the 87 relay was replaced on 9/12/18 and the 1_11 UDC 11/12 brine sensor was <br /> replaced on 8/20/18. No notification was submitted to the EHD for these replacements. No technician certifications <br /> were found on site. The following conditions are required to meet No Permit Required requirements: <br /> 1. Email EHD a scope of work by emailing to UST@sjcehd.com. <br /> 2. Notify the EHD of the time and date of the repair work by emailing to UST@sjcehd.com <br /> or fax to (209)468-3433. An EHD inspector may stop by the facility to inspect the repair and verify the technician's <br /> certifications. <br /> 3. The service company must install the part and test the system to verify functionality. <br /> 4. The service company must provide the following documentation to the tank owner/operator for inspector review <br /> during annual UST inspection: <br /> a. Description of part(s) installed <br /> b. Name& UST ICC#of Service technician doing the work (manufacturer certification proof) <br /> c. Testing done to verify functionality of the repair <br /> Immediately obtain a permit for these repairs. A'Failure to Obtain A Permit' penalty may apply. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 22, 2018. <br /> **Did not witness the full monitoring system certification. All sumps and UDCs were inspected. Witnessed <br /> testing of overfill prevention probes. Verified that vacuum sensors include vapor recovery and vent piping - <br /> sensor labels on the monitoring panel only list product piping. Unable to witness sensor functionality, leak <br /> detector testing, spill bucket testing, and communication verification. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
