The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a) Visual verification of all installed components for presence of appropriate <br /> clamps and gaskets. "All functions normal' shall be verified at the monitoring <br /> panel. <br /> b) Verification of communication inside the 87-octane fill sump interstitial <br /> 4. Since concrete will be broken over the 87-octane UST, monitoring of the secondary <br /> containment shall be verified once concrete is poured - "All functions normal' shall be <br /> verified at the monitoring panel. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br />
