As per our phone conversations,the unburied piping <br /> repairs on the UST system will fall under the 'No Permit Required—EHD Notification Required'category. The <br /> notification requirements can be found in the box located at the bottom of page 2 of the attachment, and also listed <br /> below. An EHD inspector will be on site to inspect the repairs and witness testing. Please note that due to the violations <br /> issued on October 27, 2017 for the piping leaks, all EHD staff time associated with the inspection will be billed at our <br /> current hourly rate ($152). <br /> *CONDITIONS FOR REPAIRS WHICH DO NOT REQUIRE EHD PERMIT: <br /> 1. Like-for-like replacements of parts only. Installation of different parts requires an EHD permit. <br /> 2. Email EHD a scope of work by emailing to UST@sjcehd.com. <br /> 3. Notify the EHD of the time and date of the repair work by emailing to UST@sjcehd.com or fax to (209)468-3433.An <br /> EHD inspector may stop by the facility to inspect the repair and verify the technician's certifications. <br /> 4.The service company must install the part and test the system to verify functionality. <br /> 5.The service company must provide the following documentation to the tank owner/operator for inspector review <br /> during annual UST inspection: <br /> a. Description of part(s) installed <br /> b.
