Box 338 Site Address : 2081 Country Club Blvd. <br /> French Camp, CA 95231 Stockton, CA <br /> On May 9, 1986 an investigation by the San Joaquin Local Health District staff <br /> revealed evidence of an unauthorized release of hazardous materials. <br /> In order to correct this violation, the following procedures shall be implemented: <br /> 1. Fill out the enclosed "Underground Storage Tank Unauthorized Release <br /> Report" form and submit within five (5) days if not already done. <br /> 2. Acquire the services of a qualified Engineering Consulting Firm <br /> (Hydrogeologist) to submit a plan for approval to this office for <br /> identifying the extent of contamination within 30 days. <br /> 3. The Engineering Consulting Firm (Hydrogeologist) shall submit a <br /> cleanup plan to this office for approval based upon the extent <br /> of contamination identified above. <br /> All necessary plan approvals and permits shall be obtained from the San Joaquin <br /> Local Health District prior to starting any monitoring or cleanup operations. <br /> The existing unauthorized release of hazardous material at this facility is in <br /> violation of the California Health and.
