The test boots in the 87 and 91 STP (piping) sumps were not pulled back. No Schrader valve <br /> was observed. Any potential release from the primary containment into the secondary containment in <br /> the 87 and 91 piping does not flow into a collection sump. All secondary containment systems shall be <br /> sloped so that all releases will flow to a collection sump located at the low point of the underground <br /> piping. Immediately contact a properly licensed, trained, and certified contractor to ensure that any <br /> releases in the 87 and 91 piping flows into the 87 and 91 piping sumps. <br /> 23d. The designated operator employee training for Ahmad Ahji was not performed. At the time of <br /> inspection, Mr. Ahji was the only employee on site and stated that he was unfamiliar with the UST <br /> system and the monitoring panel. The designated operator shall train facility employees for which he or <br /> she is responsible in the proper operation and maintenance of the UST system once every 12 months. <br /> Initial training shall be conducted within 30 days of hire. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response pian <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> At least one employee with current training shall be present at all times during operating hours. Ensure <br /> that employees have been trained by the designated operator, maintain the list on site, and submit a <br /> copy of the training records to the EHD by March 26, 2011. <br /> Complete & submit a copy of the Return to Compliance Certification to the EHD by March 26, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Note: The monitoring panel was found in a locked room upon arrival on site. The only employee on site <br /> did not have a key to access the room.
