Site Safety and Health Plan <br /> 3. Assure the completion of the Project Site Safety and Health Plan Briefing <br /> Acknowledgment Form (see Appendix A) by all personnel prior to their going on site, <br /> and ensuring that they understand the provisions of the form; <br /> 4. Conduct tailgate SSH meetings as required by this SSHP; <br /> 5. Assure that personnel are aware of potential hazards associated with the site; <br /> 6. Monitor the safety performance of all personnel to ensure that the required work practices <br /> are employed; <br /> 7. Immediately stop work and correct any work practices or conditions that may result in <br /> injury or unsafe conditions; <br /> 8. Conduct safety inspections and complete appropriate forms weekly(see Appendix A); <br /> 9. Prepare accident/incident reports (see Appendix A); and <br /> 10. Consult with the SSH Officer. <br /> He/she will maintain documented proof of the following requirements: <br /> 1. Copies of the Project Site Safety and Health Plan Briefing Acknowledgement Form <br /> 2. Minutes of tailgate SSH meetings <br /> 3. Copies of safety construction inspections <br /> 4. Copies of accident/incident reports <br /> 5. Records of any decisions or changes made from consultations with the Project SSH <br /> Officer <br /> 3.2 Field Workers <br /> All fieldworkers, including contractors/subcontractors, are responsible for their personal site <br /> safety and health and the safety of their co-workers. All field team members are responsible for <br /> reading and complying with this and other project SSHP(s). As required by this SSHP they are <br /> responsible for becoming aware of any hazardous site conditions, wearing the appropriate PPE, <br /> paying attention at all times, and stopping work and reporting any unsafe working conditions to <br /> the SSH Officer or Field Supervisor. No person shall perform an activity that he/she believes <br /> may endanger his/her site safety and health or the site safety and health of others. Everyone has <br /> the responsibility to stop work in the face of unsafe working conditions or practices. <br /> 3.3 Other Contractors and Subcontractors <br /> The other contractor/subcontractor site supervisors will report to the Field Supervisor. The other <br /> contractor/subcontractor site safety representatives will report to the SSH Officer. All safety <br /> representatives and officers will collaborate to ensure this and other applicable SSHPs are <br /> followed. <br /> Incident Free Operation Page 14 of 68 <br />
